*Terms & Conditions:
• Discounts are applicable for Corporate beneficiaries (Employee & their dependent family members) & Customers of IDFC First Bank Ltd.
• In case of Employee, discount shall be passed on the basis of corporate ID proof (Soft or Hard copy) & In case of Customers, discount shall be passed on the basis of Debit card/Credit Card/Passbook along with Photo ID proof.
• For dependent family members their ID proof along with copy of corporate ID card duly signed by the Employee
• Dependent family members include spouse, kids, parents & in laws.
• No discounts on Packages / TPA / ESI / Insurance / CGHS / ECHS / PSU routed cases.
• Discounts & offers are subject to revision at the sole discretion of Centre For Sight.

• Discount rate shall be determined as per prevailing Centre For Sight tariff.
